Ice Hockey Grant Assistance Request‏

Dear Lisbon Teachers, Parents, Families and Friends:

If you would like to support the continuation of the Ice Hockey team at Lisbon High School, please complete this request no later than Tuesday, November 30. 

The ice hockey teams at Lisbon and Mt. Ararat high schools need to raise money to keep the program going.  Both schools have combined resources to participate in a grant competition to win $5000, with $2,500 designated for each schools’ team.   We need as many people as possible to take a brief 10 question, self-correcting multiple choice quiz on the Responsible Sports website sponsored by Liberty Mutual.  

We need your assistance.  Please follow the quick, and easy instructions to support the ice hockey teams.

1.  Click on one of these links to take the very brief quiz.  You do not need to be a parent or coach to take the quizzes.  The questions are mostly common sense and the system will tell you if you answered incorrectly so you can just change your response, if needed.

2.  After passing the quiz you will be directed to a form page.  Fill out this very short, simple form including your name and email.  Choose ME for the state and USA Hockey for the team affiliation.  You will need to either email me Marie Stuckey at ms@stuckey.comcastbiz.net a PDF of your quiz certificate by saving it to a file, or you can just use my first name and your first name for a password so I know it and can enter your account and get a copy of the certificate.   Just let me know if you've taken the quiz so I know to find your certificate.  You will need a different email address for each quiz you take.

3.  Next you will be asked what organization to assign your credit to.  Please fill in the organization field with:  Mt. Ararat/Lisbon Ice Hockey  Fill in the city field: Topsham and state with ME.

4.  Check the email inbox for the email you provided.  Click on the link in the email from Responsible Sports to verify yourself as the quiz taker.  Without this step, the quiz will not count so please check your inbox.

5.  Log into Responsible Sports if you are not already logged on:http://www.responsiblesports.com/default.aspx  The Sign In button is on the upper right hand side of the page.  Go to your Fan Page.  The button for this is also on the upper right.  Look for certifications on the right and click on the View PDF button.  Then save the PDF to a file and email it to me or let me know I can go into your account to get it.
